An intake form provides basic information about yourself and the reason you’re seeking psychotherapy. The more information you provide, the more it facilitates our initial meeting and assists in clarifying goals.

If you have called and scheduled an appointment, you can download a confidential intake form (PDF file, free Adobe Reader required) and either bring it to your first session or mail it to Shawna V. Carboni at the address listed at the bottom of the form.
